Texas Nursing Home Guide organizes public federal nursing-home data for facilities in Texas. It is operated by AvW Consulting sro and is not Medicare, CMS, a nursing facility, a medical provider or a law firm. The directory is a research aid and does not replace a facility visit, professional advice or the current official Medicare Care Compare record.

Primary source

The directory uses datasets published by the U.S. Centers for Medicare & Medicaid Services through the CMS Provider Data Catalog. CMS describes these as the official datasets used on Medicare.gov to compare Medicare-certified skilled nursing facilities and nursing homes.

Datasets used

  • Provider Information (4pq5-n9py): identity, location, beds, ratings, staffing fields and CMS indicators.
  • Penalties (g6vv-u9sr): fines and payment denials reported by CMS.
  • Health Deficiencies (r5ix-sfxw): health-inspection citations.
  • Fire Safety Deficiencies (ifjz-ge4w): fire-safety citations and counts.
  • Ownership (y2hd-n93e): public ownership and control records.
  • Survey Summary (tbry-pc2d): survey-cycle summaries.
  • Medicare Claims Quality Measures (ijh5-nb2v): claims-based resident outcome measures.
  • MDS Quality Measures (djen-97ju): resident-assessment quality measures.
  • FY 2026 SNF VBP Facility-Level Dataset (284v-j9fz): annual value-based purchasing fields where displayed.

Monthly update and validation

The automated updater is scheduled monthly. It requests the complete Texas set for every required CMS dataset, rejects incomplete downloads, requires unique CMS Certification Numbers, validates facility and county totals, and builds a staged release. Publication occurs only after validation and uses locking, verified backups and rollback. A page's displayed date identifies the CMS data snapshot used for that release; it is not necessarily the date every underlying event occurred.

Facility identity and URLs

CMS Certification Number (CCN) is the primary facility identifier. Uppercase names and locations are normalized to readable title case when appropriate without changing the CCN or numeric values. A facility keeps a stable clean URL where possible, and older URLs redirect to its current canonical page.

CMS ratings

Overall, health-inspection, staffing and quality-measure star ratings are CMS ratings. They are not visitor reviews and are not calculated by this website. CMS assigns one to five stars and cautions that no rating system covers every factor important to choosing a home. See the official CMS Five-Star Quality Rating System.

Texas and county comparisons are descriptive calculations from facilities with usable values in the same validated snapshot. Missing comparisons can mean too few facilities reported a usable value.

Inspections, penalties and CMS indicators

Inspection citations, fines, payment denials, Special Focus status and the CMS abuse icon come from CMS fields or identified CMS datasets. These labels do not represent an independent accusation or finding by this website. Citations can be subject to CMS dispute processes, and status can change in a later release. Check the current official Care Compare record before making a decision.

Staffing and outcome measures

Staffing values are displayed as CMS publishes them and may include hours per resident day and turnover. Claims and MDS measures cover specified reporting periods that may end before the site snapshot date. Units and reporting periods are displayed where available.

Limitations

  • Published data can lag current facility conditions.
  • Missing data does not establish that an event did or did not occur.
  • CMS ratings do not cover every aspect of care or personal fit.
  • County and state comparisons do not establish causation.
  • Automated title-case normalization may require correction for an unusual proper name.
  • This website does not independently inspect facilities.

Corrections

Use the private contact form and choose “Data correction.” Include the facility name, CCN, page URL, disputed field and a supporting public link or description. When a value comes directly from CMS, the site may retain the published CMS value while adding context or directing the requester to the CMS correction process.

Editorial and commercial separation

Directory ordering and CMS data are separate from attorney advertising or case-review services. Paid relationships do not change facility ratings, inspection records or comparisons. See our Advertising & Attorney Disclosure.
